| 内容記述 | The purpose of this study is to clarify problems and stresses which the nurses of hospices and palliative care units have. The subjects are 381 nurses who work in hospices and palliative care units. Data were collected, using two types of questionnaire: the selection type and the free description type. The nurses experienced scene of the sedation (95%), sedation order of the doctors in absence (67%), changing the quantity of the sedative by their judgment (74%), the patients' breath stopped after having started sedation (40%), the need to inform the patient necessity of sedation with getting consent by themselves (86%), problems and stresses for the sedation procedure (78%). The qualitative functional analysis revealed that problems and stresses reported by nurses could be classified into three categories. The 1st category was "the problems and stresses about sedation itself" which consists of "the connection with life and death", "the ambivalence pertains to sedation" and "the negative image of sedation". The 2nd category was "the relation between problems and stresses about the sedative effectuation" which consists of "the difficulty in explanation and the difficulty to make certain of intention", "the patients and their families can't get together", "the difficulty and imposition when nurses actually did" and "the difficulty when they care about families". The final category was "the problems and stresses relating to the organization and medical system" which consists of "the problems among medical staff" and "insufficient education and system". | |||||||||||||||||||
