| 内容記述 | 舌弁は1909年, Lexerが頬部の組織欠損に使用したのが始まりで, また1966年にはGuerrero Santosらが鼻口腔瘻の閉鎖に舌弁を使用している。この頃より舌弁は盛んに用いられるようになり, 現在では口腔, および中咽頭領域の比較的小さな再建や鼻口腔瘻閉鎖術の1手段として有効な地位を保っている。今回われわれは, 当科にて行った舌弁症例について臨床的検討を行ったので報告した。対象は1980年から1999年に東北大学歯学部附属病院第二口腔外科にて舌弁を使用した11症例であった。性別は男性5人, 女性6人。年齢は13歳から82歳で平均年齢は34歳であった。症例の内訳は鼻口腔瘻閉鎖が最も多く8例, 放射線照射後の骨の被覆に用いた症例が2例, 赤唇形成症例が1例であった。11例中2例に皮弁の一部離開が認められた。離開症例は, いずれも瘻孔閉鎖症例で茎部を舌根部とした側方舌弁であった。舌弁は豊富な血管が存在し生着率も高いことより, 口腔内の組織欠損において症例を適宜選択すれば有効な方法と思われた。 | |||||||||||||||||

| 内容記述 | The use of a tongue flap was first described in 1909 by Lexer, who used this flap to reconstruct a defect of the buccal mucosa. Guerrero Santos reported in 1966 that he used a tongue flap to repair a palatal fistula. Subsequently, tongue flaps have been used to treat various conditions, including palatal fistulas and oral tissue defects. Tongue flaps are still used to repair small defects of oral or oropharyngeal tissue. We describe the clinical use of tongue flaps in 11 patients(5 men and 6 women, mean age 34 years) treated at the clinical division of Oral and Maxillofacial Surgery II, Tohoku University Dental Hospital between 1980 and 1999. Eight patients had palatal fistula, 2 had recurrent cancer treated by radiation and I under went lip reconstruction after tumor resection. Suture failure occurred in 2 patients in whom lateral tongue flaps based on the posterior part of the tongue were used to close palatal fistulas, but necrosis did not develop. Our results show that a tongue flap is useful for the repair of oral tissue defects. | |||||||||||||||||
